A 38-year-old man died Monday after he was pinned by a forklift while working at a cold storage warehouse in Clearfield, police announced Tuesday.

The Clearfield Dispatch Center received a 911 call around 12:30 p.m. Monday reporting that an employee at Americold Logistics was pinned by a forklift and bleeding, a news release states. The warehouse is located at 755 E. 1700 South in Clearfield.

When North Davis Fire District crews and Clearfield police officers arrived at the scene, they found the employee pinned between the forklift and some freezer shelves, police said.

They were able to free the man, and first responders provided medical aid at the scene, but he had suffered “significant trauma” to his lower extremities, the release stated.

The man was taken to a hospital, but he died a short time later. He was from Sunset, police said, but officials did not name him Tuesday, pending notification of family members.

“Initial information suggests this was a tragic accident,” police said in the news release.

Officials with Utah Occupational Safety and Health Division were notified of the workplace death, police said. A spokesperson with the Utah Labor Commission confirmed that the workplace safety division is investigating whether any safety violations may have contributed to the fatality.

“The Clearfield Police Department expresses sincere condolences to the victim’s family, co-workers, and all those affected by this tragedy,” the police department said in a statement.

Americold builds temperature-controlled food supply chain infrastructure and is based in Atlanta, Georgia, according to its website. “Americold’s facilities are an integral component of the supply chain connecting food producers, processors, distributors and retailers to consumers,” the website states.

Efforts to reach Americold for comment Tuesday were unsuccessful.
