On Sept. 18, 2014, the vehicle drove off a bridge near 1040 W. Indiana Ave. and crashed into the Jordan River upside down, trapping three occupants. Leo Montoya Jr., Johnny Dawn and Adrian Lyon were the first on the scene to enter the river and free the occupants. They have been awarded Citizen Heroism Medals by Salt Lake City Fire Chief Brian Dale. A plaque unveiled Wednesday honors them and the others who prevented tragedy.
