The Utah Highway Patrol has identified a man and his young daughter who died after being rear-ended on Interstate 15 in Weber County this weekend.

The accident occurred about 5:30 p.m. Sunday as troopers were trying to clear debris from the area were the freeway interchanges with Interstate 84, near Riverdale. A Mazda with a mother, father and two children had stopped behind a semi-trailer there when a Chevrolet sedan hit it from behind.

The collision propelled the victims' car into the back of the semi.

Declared dead at the scene was Ryan Chambers, 30, of Layton; his 4-year-old daughter, Evelynn Chambers, died at the hospital a short time later.

The mother, 28-year-old Alyssa Chambers, had been upgraded to serious condition as of Monday; her 7-year-old daughter, Kaya Chambers, remained in critical condition at Primary Children's Hospital in Salt Lake City, UHP Sgt. Todd Royce said.

The driver of the Chevrolet, Uchenna Uzoh, 32, of Ogden, was not injured.

No citations had been issued as the investigation was ongoing. UHP particularly wanted to identify the owner of a dog house that fell off a vehicle onto the freeway, causing the debris that contributed to the accident scene.
