A Utah Department of Health program that provides insurance for some low-income Utahns is set to open to new enrollees beginning Monday.
The Primary Care Network (PCN) provides coverage for uninsured adults ages 19 to 64 with or without children who lack access to Medicaid, Medicare or veteran-related health care. To qualify for the program, enrollees must be U.S. citizens and meet specific income requirements.
A single person, for example, can earn no more than 150 percent of the poverty level, or $17,235. A family of four could earn no more than $35,325 annually.
PCN coverage provides limited preventative coverage, including doctor visits, prescriptions, dental care, vision care, immunizations and other services.
Enrollment in the program costs $50 annually, according to the health department. Applications for the program are available through the department's web site, http://www.health.utah.pcn.
The enrollment period for individuals is open from April 22 to May 6. For families, the enrollment period runs through May 17.
