The teen driver of a vehicle that may have been the catalyst for a double-fatal, multi-vehicle crash in Tooele County Tuesday has been questioned, but investigators remain undecided on charges.
Utah Highway Patrol Cpl. Todd Johnson said Thursday that the 16-year-old male, who contacted UHP after hearing about Tuesday’s crash on State Road 36 at 8793 N. Lakepoint, had been "fully cooperative" with troopers.

"He indicated that he was aware of the accident but thought it was well behind him when it occurred," Johnson said.
Johnson could not discuss further details due to the ongoing investigation. However, Johnson indicated that earlier reports that the boy’s black SUV may have caused Sloane F. Ackman to suddenly brake, triggering two more collisions, remained in question. The boy had left the scene by the time troopers arrived.
"Our investigators will be re-interviewing all the witnesses about what happened. After that, they will decide what, if any, charges will be filed," Johnson said.
It will be well into next week before that determination is made, he added.
Ackman, 49, and her daughter, Trista S. Ackman, 29, were pronounced dead at the scene. The Ackmans were driving their station wagon northbound on SR-36 at 4 p.m. when they suddenly braked, causing a truck to rear-end them; the impact propelled the Ackmans’ car head-on into a southbound SUV.
Neither woman was wearing a seat belt, the UHP said.
