Product • Imaginarium 5-Sided Activity Center, sold at Toys "R" Us stores.

Problem • The small wooden knobs attaching the xylophone keys to the end can detach, causing a choking hazard to young children. Consumers should immediately stop using the product, move it out of the reach of young children and return it to a Toys "R" Us store for a refund or store credit. Contact Toys "R" Us toll-free at 800-869-7787 from 7 a.m. to 9 p.m. Monday through Saturday, and from 9 a.m. to 5 p.m. Sunday.

Vehicle • 2012 Honda Civic

Problem • During assembly, the process required to seat the driver's-side driveshaft and set the retaining clip was not completed. As a result, the driveshaft may separate, causing a loss of drive power that may result in the vehicle rolling if the parking brake has not been set. Honda will notify owners, and dealers will inspect the driver's-side driveshaft and replace defective ones free of charge. Owners may contact Honda Customer Service at 1-800-999-1009.
