Police have identified a 37-year-old man who died in a three-vehicle crash Tuesday afternoon in Clearfield.

Clearfield police Lt. Kelly Bennett said that the victim, Casey David Archuleta, of West Point, was driving a eastbound Nissan Sentra when he was struck about 4:30 p.m. by an out-of-control, westbound Dodge Dakota near State Road 193 and Center Street.

Bennett said that a moment before, the Dakota had been rear-ended by a Ford F-250 pickup truck making a turn onto SR-193.

Archuleta was rushed to Davis Hospital and Medical Center, where he later died of his injuries. Two passengers in the Nissan, Archuleta's 34-year-old wife, Melissa, and their 13-year-old daughter, whose name was not released, were treated at the hospital and released.

Also transported to the hospital and later released was the driver of the Dakota, a 26-year-old Sunset man. The driver of the Ford, an 18-year-old man from Layton, was treated at the scene for minor injuries.

Bennett said that both Clearfield police and the Utah Highway Patrol were investigating the accident. As the conclusion of their inquiries, charges may be screened.
