A Salt Lake City man died after the car he was working on apparently fell off the jack and crushed him Wednesday night.

Neighbors found 49-year-old Mario Perez pinned beneath a Volkswagen Passat at his home on the 1700 West block of Kimberly Circle (460 North) and called the Salt Lake City Police Department at 5:55 p.m., according to Lt. Robin Heiden.

"The jack either failed or slipped out; we're not sure yet. The car came down on top of him," Heiden said. "It was a tragic, tragic situation."

The neighbors got another jack and lifted the car off the man. First responders took the man to Intermountain Medical Center, where he was pronounced dead.
