A 24-year-old Salt Lake City woman whose 3-year-old unbuckled daughter died last year during a traffic accident has been charged with negligent homicide.

Carolyn Dawn Hughes was charged last month in 3rd District Court with a class-A misdemeanor for the death of Paisley Hayden, who was ejected from Hughes' SUV during the Sept. 3 crash.

Hughes' SUV was in a northbound left-turn lane at the intersection of Redwood Road and Research Way, at about 2700 South, and carrying eight children from multiple families in the vehicle when the crash occurred.

She later told police some of the children, including Hayden, were not wearing seat belts, according to charging documents. Later it was revealed that no one in the SUV was wearing a seat belt, including Hughes.

A southbound vehicle collided with Hughes' SUV on the passenger's side when she attempted to make a left turn. She said she didn't realize the oncoming car, which was going about 40 miles an hour, was "going so fast," according to charges.

The 3-year-old was ejected through a window on the driver's side and was then crushed by the overturning SUV.

Hughes' 2-year-old son suffered critical injuries to his leg from the crash and was taken to the hospital.

Neither Hayden nor her brother were in car seats.

In 2014 – prior to the fatal collision – court records show that Hughes was ticketed by a Utah Highway Patrol officer in Summit County for failure to use a child restraint device and fined $95.

The crime she now faces is punishable by up to a year in jail.

A scheduling conference for Hughes is set for May 13.
